Job responsibilities
Primary responsibilities
Manage chronic diseases, including diagnosis, monitoring, and individual care planning
Review patients with conditions such as diabetes, hypertension and heart disease, adjusting care plans to improve health
Assess lifestyle and medications, recommending changes where appropriate
Use evidence-based processes to manage longterm conditions effectively
Identify and support patients at risk of developing long-term conditions
Perform venepuncture and manage specimens
Carry out diagnostic tests and screening within practice protocols (e.g. ECGs, cervical cytology)
Provide immunisations and vaccinations for children, adults and travellers
Undertake ear examinations and irrigation
Provide wound care, dressings and suture removal
Deliver first aid and manage emergencies (e.g. burns, bleeding, shock, cardiac arrest), including triage
Maintain clinical equipment appropriately
Complete new patient health checks
Signpost patients to appropriate clinicians or external services, ensuring continuity of care
Use systematic triage and holistic assessment to promote highquality, personcentred care
Maintain accurate, clear clinical documentation
Ensure arrangements for patient care are completed promptly and safely
Communicate effectively with multidisciplinary teams
Maintain patient confidentiality in line with NMC guidance
Clearly communicate complex or sensitive information to patients and carers, recognising barriers to understanding
Promote positive patient experiences through courtesy and discretion
Support patients with health promotion, illness prevention and selfmanagement
Recognise and act on situations posing risk to patient wellbeing
Ensure care is evidencebased and in line with local policies
Provide opportunistic health promotion where appropriate
Follow practice and local policies on substance misuse, referring patients appropriately
Clinical Practice Leadership
Contribute to clinical audits and support the development and monitoring of care standards.
Support and teach colleagues in the clinical environment, including unqualified staff.
Act as a mentor or assessor for team members when required.
Work collaboratively and effectively within the multidisciplinary team.

Experience
Essential
- Broad knowledge of general practice and experience working as a Practice or Community Nurse
- Knowledge of chronic disease management
- Experience of Practice Nurse skills including wound care, ECG, venepuncture, immunisations, new patient medicals and womens health including cervical cytology
- Strong clinical IT skills with the ability to record accurate and detailed clinical notes
- Able to communicate complex or sensitive information effectively by phone, email and in person
- Good understanding of safeguarding for both adults and children
- Ability to work independently within boundaries
- Ability to work as part of an integrated multi-skilled team
Desirable
- Experience of SystmOne clinical system
Qualifications
Essential
- Registered General Nurse
- Recognised qualification to diploma level or above
- A demonstrable commitment to professional development
Desirable
- Has undertaken specific training relevant to Practice Nursing e.g. Diabetes or Respiratory

- Work autonomously and manage their own area and workload.
- Have periods of working independently in isolation
- Require high levels of motivation and a positive approach.
