A Band 5 Community Nurse provides essential nursing care to patients in their own homes and local community settings, supporting individuals with a wide range of health needs. The role involves assessing patients, planning and delivering care such as wound management, medication administration, and support for long-term conditions, while promoting independence and wellbeing. End of life care is a key part of this role in delivering compassionate care for patients at end of life alongside care and empathy for families. Working both independently and as part of a multidisciplinary team, the nurse collaborates closely with colleagues including GPs, therapists, and social care services to ensure coordinated, safe, and effective care. Nurses should be able to input and retrieve data effectively, maintain confidentiality, and follow information governance standards. Key skills include problem-solving, decision-making, time management, and the ability to assess patients, prioritise care, and make appropriate referrals using evidence-based practice. The role also demands excellent interpersonal and organisational skills, the ability to work under pressure, adapt to changing environments, maintain accurate records, and work within organisational policies.
